As  passionate and certified Mental Health Counselors, our experience allows us to understand the best ways to address each client’s needs in an effective manner.  Our main focus is to help our clients develop a leader spirit where they can take charge , overcome, and change the course of their lives for the better!

Initial diagnostic Evaluation

  • A diagnostic assessment is a clinical evaluation provided by a licensed professional in order to gather information to determine appropriate treatment based on the initial problem, current mental status and the diagnostic impression. The clinician will share the results of the diagnostic assessment with you in order to begin treatment planning.

50-53 min Follow up visits

This is  a visit to a mental care provider after the initial consultation and treatment. The focus is on the management, treatment, and prevention of various mental disorders. In addition, clients have an opportunity to process their thoughts, feeling, and behaviors and gain useful coping skills and strategies to assist them with achieving their goals for treatment.

53-60 min Crisis session

Crisis intervention is a short-term (usually single session) technique used to address an immediate mental health emergency, stabilize the individual in crisis, and create and implement a safe, appropriate plan for next steps and future treatment.

55-60 min family session

Family therapy is a type of psychological counseling (psychotherapy) that can help family members improve communication and resolve conflicts. Family therapy is often short term. It may include all family members or just those able or willing to participate. Your specific treatment plan will depend on your family's situation. Family therapy sessions can teach you skills to deepen family connections and get through stressful times, even after you're done going to therapy sessions.

 53-60 min Couples/Marriage Session

Couples therapy is a form of psychotherapy that can help you and your partner improve your relationship. Couples therapy can address a wide range of relationship issues, including recurring conflicts, feelings of disconnection, an affair, issues related to sex, or difficulties due to external stressors

45-60 min Pre/post Marital Education

This is not the same as pre-marital counseling/couples counseling. This is a 10 week course to educate and help clients began to identify areas of marriage that often go unaddressed prior to saying "I do" and jump start working to build a healthy foundation for a lasting marriage. THIS SERVICE IS PRIVATE PAY ONLY!

                                                              Discipline and Complaints

 

The Council reviews all complaints received by the agency for potential violations of state laws and rules. The Council has implemented a priority system to quickly identify and investigate complaints that involve particular threat of harm to the public. In addition, consumers have a right to file a consumer complaint if a business or provider has violated their rights. SECTION 165.2. Medical Record Release and Charges

Deadline for Release of Records. The requested copies of medical and/or billing records or a summary or narrative of the records shall be furnished by the provider or facility within 15 business days after the date of receipt of the request and reasonable fees for furnishing the information.

A reasonable fee for providing the requested records in paper format shall be a charge of no more than $25 for the first twenty pages and $.50 per page for every copy thereafter.

Records may be collected by client or person/agent with a completed Release of information authorizing CHART Counseling Services on their behalf to provide records with specifics to what can and cannot be provided to the requested party. 

The counselor or facility responding to a request for such information to be provided in electronic format shall be entitled to receive a reasonable, cost-based fee for providing the requested information in electronic format. This fee may be subject to a reasonable fee for providing the requested records in electronic format shall be a charge of no more than: $25 for 500 pages or less; $50 for more than 500 pages.

Letters to 3rd party entities are separate and have a separate cost. In addition, Family Medical Leave Act (FMLA), short-term disability (STD), and long-term disability (LTD) are not considered part of a client’s medical records and therefore have separate costs and conditions. This service is NOT covered by insurance. This service is done at the discretion of the provider. Please discuss  plans to seek a leave of absence (LOA) from employment with your provider prior as it is not a guarantee that provider will complete the required paperwork.  

     

If an affidavit is requested, certifying that the information is a true and correct copy of the records, whether in paper, electronic or hybrid format, a reasonable fee of up to $15 may be charged for executing the affidavit.
